Efficacy of testosterone therapy on total penile length and penile length-to-meatus ratio in children with proximal penile hypospadias

Abstract

Introduction: Efficacy of testosterone therapy is crucial for the treatment among children with proximal penile hypospadias. The purpose of the study was to observe and compare the changes in the penile length before and after IM testosterone therapy.

Materials and methods: This was a pretest-posttest study conducted in the Surgery Outpatient department (SOPD) of Bangladesh Shishu Hospital & Institute, from November 2022 to October 2024, over a period of 24 months. Patients diagnosed with proximal penile hypospadias at SOPD during the study period were recruited. Several variables were recorded like total length of the penis, proximal penile length (penile length peno-scrotal junction to meatus), distal penile length (penile length meatus to tip of the glans), ratio between proximal penile length and distal penile length along with glans width. IM testosterone injection was administered to each patient at a dosage of 2 mg/kg of body weight. If the desired effect was achieved, the testosterone therapy was discontinued.

Results: A total of 25 children with proximal penile hypospadias were recruited, with a mean age of 39.64 ± 26.46 months. During testosterone therapy, the mean total penile length was 19.76 ± 4.96 mm before and 29.08 ± 4.36 mm after intramuscular testosterone injection (p = 0.0001). The mean average proximal penile length was 4.88 ± 2.54 mm before and 12.16 ± 3.86 mm after intramuscular testosterone injection (p = 0.0001). Following testosterone therapy, the mean ratio between proximal penile length (AB) and distal penile length (BC) changes from 0.34±0.20 mm to 0.77±0.38 mm (p=0.0001).

Conclusion: In conclusion, intramuscular testosterone therapy plays a significant role in promoting growth in total penile length, particularly from peno-scrotal junction to meatal length, as well as improving the ratio between proximal and distal penile length.
