Psycho-social factors of eating disorders in children with cancer
Abstract
Eating disorders in pediatric oncology are well described along with other somatic, behavioral and emotional effects of antitumor treatment and need more intensive investigation. The medical factors of the formation of eating disorders have been well studied. However, psychological and medical factors remain poorly understood.
The goal was to study psycho-social factors of eating disorders in children with various malignant tumors.
Material and methods. 77 children aged 4-17 (average 8,6) with hemoblastoses (31%), brain tumors (34%), other solid tumors (35%) in remission of 1-7 years (average 4) were included in the study. The methods consisted of the analysis of projective and verbal material of a child, clinical observation, clinical interview with the parents, analysis of the questionnaires filled by one of the parents. All studied children received at least 10 psychotherapy sessions. Statistical analysis: SPSS statistics 17,0.
Results. Changes in eating behavior were detected in 56 (73%) children: overeating in 14% including vomiting in 4 children; food avoidance or restriction in 48% including vomiting in 11 of them; selectivity in food in 38% including vomiting in 5 of them. 45% of
the sample demonstrated depressive reactions to anxiety, 75% - autoaggression. Statistically significant connection of eating disorders with any gender or any of the nosological groups was not found. However, it was shown that children with BT are at an increased risk of forming persistent eating disorders. Psychological and social factors of eating disorders in children have been identified.
Conclusion. Eating disorders in children with cancer exacerbate existing problems, complicate the treatment and rehabilitation of the patient, therefore they require proper assessment and treatment by specialists at the pediatric oncology clinic.
