Primary disability of children due to cerebral palsy in the Russian Federation
Abstract
Disability is an important medical and social indicator of health. Disability indicators make it possible to characterize the level of socio-economic development of society, the state of the living environment, the degree of perfection of medical care, and the quality of ongoing therapeutic and preventive measures. The relevance of this problem is due, first of all, to its significant scale and negative dynamics. An important role belongs to the development of medical, social, and rehabilitation measures to eliminate, correct and prevent disability among the child population.
In our study, we identified the following patterns in the analysis of indicators of the number, level and structure of childhood disability due to cerebral palsy :
– a period was identified of a decrease in the number of primary disabilities of children from 447 people in 2015 to 228 in 2021 and a period of increase in the number from 284 in 2022 to 289 in 2023, respectively, in the Russian Federation.
– in the Russian Federation, children were diagnosed with disabilities quite consistently – from 93.0% to 89.7% of those referred; in Moscow, the proportion of children who were diagnosed with a disability when contacting bureau decreased to 82.0%, in other words, only 8 out of 10 children were diagnosed with a disability in the Moscow bureau;
– a decrease in the share of primary childhood disability due to cerebral palsy among all causes of disability with a high degree of reliability, which is confirmed by approximation coefficients in Moscow (R 2 =0.7899) and in Russia (R 2 =0.9529);
– a positive close relationship has been established between indicators of primary disability and primary incidence of cerebral palsy in children, which is confirmed by the K. Pearson coefficient +0.95; there is no correlation between the average annual number of children and primary disability due to cerebral palsy;
- in Moscow, boys predominated 1.4 times, their share was 58.7%, the share of girls was 41.3%, respectively; Similar dynamics have developed in the Russian Federation;
– in Moscow, 80.7% were diagnosed with disability for the first time before the age of 3 years, without significant differences from the Russian Federation
– in 19.3% of children, disability was first diagnosed after 3 years of age, in preschool or school age, which may be due to a later application to a medical organization and medical institution, underexamination of children due to low parental responsibility, failure of compensation and progression symptoms due to school load with persistent minor dysfunctions in the child;
Thus, the results of the comparative analysis are important for organizing the work of medical and social examination and medical rehabilitation of children with cerebral palsy

Bella Gazdieva graduated from the Medical University named after N.I. Pirogov in Moscow, majoring in pediatrics, then completed residency in neurology, then completed postgraduate studies in the following specialties: public health and healthcare organization, and neurology.
